DESCRIPTION OF WORK
Assesses assigned residents' psychological and social needs, admission, and discharge planning, provides emotional support, and coordinate the delivery of social services to the resident to attain the highest practicable physical, mental, or psychosocial well-being. Work follows Federal, state, and local standards, guidelines, and regulations governing the facility, and as directed by the Caseworker Supervisor to ensure that the highest degree of quality resident care is always maintained.
Job Details
- Establishes relationships with and advocates for residents or their families.
- Performs required documentation on MOS, medical charts, and departmental folders, including summaries, reviews, and progress notes.
- Participates with the Center's psychiatric psychological and MR consultants reporting on residents' status assisting in implementing recommended treatment and informing psychologists of new referrals.
- Participates in resident care conferences.
- Provides counseling to residents and their family members.
- Performs admission procedures such as reviewing administrative packets with residents or their families, assists in getting telephone and television service, compiles social history, reviews advance directive procedures, and provides orientation to the building and its services.
- Coordinates discharge meetings, and all services needed after discharge such as: housing, furniture, household goods, home health equipment, visiting nurses, community support, and hospice care.
- Coordinates guardianship proceedings, psychological commitments, hospice referrals, and support to dying or ill residents and grieving families.
- Completes documents such as living wills and power of attorney.
- Assists in restraint reduction program.
- It helps residents and family members obtain specialized services through community agencies and agents, including access to transportation and durable medical equipment.
- Performs related tasks and duties as assigned by the Caseworker Supervisor.
Requirements
- A bachelor's degree in social work, psychology, or another human service-related field is required.
- Minimum 2-5 years experience in social work or related field.
